Waiver-Release of Liability

WAIVER-RELEASE OF LIABILITY

In consideration for my dog boarding at Tikaani-Atka Boarding Kennel for day and/or overnight, I agree to all the following policies, procedures, terms and conditions.

Please initial next to every point to indicate that you have read and understand every policy, procedure, term and condition of this agreements.

PROPERTY: Tikaani-Atka Boarding Kennel shall not be responsible for any lost, stolen or damaged personal property belonging to my dog or myself. If my dog causes damage to the Tikaani-Atka Boarding Kennel facilities or equipment, I agree to be fully responsible for the full cost of repair or replacement. ____

BOARDING AND DAYCARE FEES: I understand that boarding is on a 24 hours system and daycare is between the hours of 7:30am-5.30pm.  I agree to pay for all boarding fees, services, or products at the time I pick up my dog after each visit and that in the event that I or my authorized contact cannot pick up my dog(s) at the agreed pickup time, I authorize Tikaani-Atka Boarding Kennel to provide additional overnight or daycare services at my expense. ___

BOARDING AND DAYCARE PARTICIPATION RISK: Tikaani-Atka Boarding Kennel, owners and staffs will do the utmost to ensure the safety of your dog. There are potential risks involved with interactions between humans and dogs, as well as between dogs and other dogs, which may result in injury or death. There may be risks not known to you nor readily foreseeable during your dog’s stay. I fully accept and assume responsibility for risks, including, without limitation all losses, costs and damages incurred as a result of my dog’s stay at Tikaani-Atka Boarding Kennel. ____

PET AGGRESSION:  I understand that Tikkani-Atka Boarding Kennel reserves the right to remove my dog from a play area and place my dog(s) in a separate holding area should my pet(s) display any unwanted behaviors. ____

VACCINATIONS: I understand that while my dog is fully vaccinated with Canine distemper, rabies and Bordetella (kennel cough) that vaccines are not guaranteed and there is a small risk that my dog might contact a contagious disease or illness. I agree that should this occur, I am responsible for my pet’s care medications and costs. ____

VETERINARIAN CARE: I agree to allow Tikaani-Atka Boarding Kennel to obtain medical treatment for my dog if he/she is ill, injured or exhibits behavior that would suggest that my dog may need medical treatment. I agree that I am fully responsible for the cost of any such treatment. ____

WAIVER AND RELEASE OF LIABILITY: I release from liability, hold harmless, and agree not to hold Tikaani-Atka Boarding Kennel and owner responsible for any and all injuries, escapes, damages, and any other costs associated with my dog’s stay and participation in activities at Tikaani-Atka Boarding Kennel. ____

I agree that this agreement shall apply to all boarding and daycare visit by my dog at Tikaani-Atka Boarding Kennel. ____
